We're almost there! The Disciple Maker Index survey becomes active on Thursday of this week!!! Starting on Thursday, click the link of the parish you are registered at and complete the survey. Paper surveys will be available for those without internet access by contacting the Pastorate office (563-870-4041).

The St. John Baptist de La Salle Pastorate is on a journey to know, love, and serve Christ in our Pastorate community. Together we must embrace Christ’s vision for missionary discipleship so our parish can thrive and be a place for others to grow together in faith.
The very first step in this process involves assessing our greatest strengths and opportunities, and that is where the Disciple Maker Index (DMI) comes in! The DMI is an important survey tool that invites you to reflect on your discipleship journey here at the St. John John Baptist de La Salle Pastorate and the parish you are a member of.
The Disciple Maker Index is designed to:
.- Invite parishioners to reflect on their journey of discipleship.
- Enable parish leadership to make data-driven decisions.
- Help parishioners grow in their faith.
The 10–15-minute survey will be accessed through online links found on this page for your parish and will ask you to reflect on your own spiritual growth and enable you to provide your perspective on your parish's efforts to help you grow in relationship with Christ. All responses will be confidential, and the parish will only receive information about the community as a whole. The link to each parish's survey is located on this page. Paper surveys will also be available for those who do not have the ability to complete the survey online. Contact the Pastorate office and ask that a survey paper copy be printed for you.

Frequently Asked Questions
How was the Disciple Maker Index survey created?
Catholic Leadership Institute gathered 100 bishops, priests, diocesan officials, parish leaders and authors/thought leaders (Sherry Weddell, Matthew Kelly, etc.) to a 2-day “think tank” on parish engagement which produced some initial hypotheses. CLI then took a smaller group of the same constituents and crafted the survey. After that, CLI shared the survey with 15 bishops for feedback and edit. The survey was first administered in 2014 and to date has received more than 600,000 responses.
How do I access the Disciple Maker Index survey?
You access the online version of the survey via an online link that will be provided to the parish [and found on this page]. Note that this survey will only be active during the launch time. If you do not have internet access, ask a friend or relative who does and have them read you the questions, you let them know your responses, and they will submit the survey once all questions are answered. A paper version of the survey will also be made available to the parish by contacting the Pastorate office (563-870-4041).
How long does the survey take to complete?
10-15 minutes
What is the youngest age able to complete the survey?
Keeping in mind local diocesan and civic policies, Catholic Leadership Institute recommends that only individuals 18 years of age and older complete the survey. [The Archdiocese of Dubuque will be following this recommendation.]
